Sonic the Hedgehog 3 poursuit l'histoire après Sonic 2, avec Sonic rencontrant Knuckles, le gardien de l'Émeraude Maîtresse, qui est trompé par Dr. Robotnik en croyant que Sonic est l'ennemi. Après une bataille, Robotnik répare l'Egg de la Mort et vole l'Émeraude Maîtresse, poussant Knuckles à rejoindre Sonic pour la récupérer. Les joueurs peuvent choisir entre Sonic et Tails en mode solo ou coopératif, naviguant à travers des obstacles et combattant les robots de Robotnik. Le spin dash permet des attaques rapides, et collecter des anneaux sert de bouclier contre les dommages, avec des niveaux spéciaux accessibles aux checkpoints après avoir rassemblé 50 anneaux.
